APPLICATION FOR PENSION

To the Honorable County Court of Scott County
Sitting as a Pension Board:
The undersigned, as an ex-Confederate soldier, hereby applies for a pension under the provisions of an act of the General Assembly of the State of Arkansas, entitled "An Act for the Relief of Certain Soldiers of the late War between the States," approved April 1, 1891, upon the following grounds:
That he is a bona fide citizen of the State of Arkansas, and has resided within the State for more than twelve months next before the filing of this application;
That he enlisted as a soldier in the State of Alabama during the war between the States in Company "G" of the 41st Regiment of Alabama Vols.
That while in the discharge of his duty as a soldier of said company and regiment in the service of the State of Arkansas or of the Confederate States of America he received a wound from the effects of which he lost his left arm above the elbow and about one inch below left shoulder.
That by reason of said wound and disability he is now indigent and incapacitated for manual labor;
That he is not receiving any aid from the State or from the United States for such injuries under any other statute, and that therefore he makes application for relief under the act aforesaid.

Mr. Thomas M. Duncan your communication Rec'd. concerning Thomas Sudduth pension. I can't testify to the affidavit. Thomas Sudduth was known to me from boyhood he was a private in Co. (G) 41st Ala. Vols. he was a good soldier & is full worthy of a pension I don't Rember [sic: remember] what battle he was wounded in. I have not known him for 20 years I only have saw him once during that time. I know he was wounded in the battle and ought to have help. I am willing to do anything I can to help him free of charge. I am engaged now in that business[?] when called on free of charge.

Duncan, County Clerk of Scott County, consituting the pension board of said County, by an Act of the General Assembly of the State of Arkansas, approved April 1, 1891, entitled "An Act for the relief of Certain Soldiers of the late War between the States," do hereby certify to the Auditor of the State of Arkansas that upon examination of the application of Thomas Sudduth filed under said act, we find that said claim of said applicant is just and the same is hereby allowed, the application and proof taken being herewith transmitted.

APPLICATION FOR PENSION

I, Thomas M. Sudduth, do solemnly swear that I served as a soldier in the army of the Confederate States, being a member of Company "G" commanded by Capt. Hudgens first & afterwards White, in Col. Talbot's 41st Regiment of Infantry Vols. from the State of Alabama; that I was honorably released while in hospital at Farnsville[?] Va. from such service on or about the 23rd day of May 1865, and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life and that such incapacity (or disability) is the result of wounds received in the service, being gun shot wound in left arm, my left arm being off within an inch or inch and one half of left shoulder joint - and on account of weak lungs and lung disease which renders me almost unable to speak at times, also spinal affliction and advanced age, my age now being 62 years, and my debilitated physical condition and that such disability is not the result of my own vicious habits still persisted in, so help me God.

On this day personally came before the undersigned, a Justice of the Peace within and for the County of Lamar and State of Alabama, David Sudduth, a citizen of Canada[?], Ala. whom I certify to be a creditable person and worthy of confidence, who being duly sworn, states that he is personally well acquainted with applicant Thomas M. Sudduth having known him for 50 years.
That he was a Confederate soldier. Belonging to Company "G" 41st Regiment of Ala. Infty. Vols. That as such soldier he served from Dec. 1862 to the close of the war in the spring of 1865. That he was honorably released from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of gun shot wound in left shoulder, his left arm being off close to left shoulder joint, and old age, and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife to the best of my knowledge and belief is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That I have no interest in this claim. That I served in same Company and Regiment with soldier.

That he was a Confederate soldier. Belonging to Company "G" 41st Regiment of Ala. Infty. Vols. That as such soldier he served from May 1862 to the close of the war in spring of 1865. That he was honorably released from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of gun shot wound in left arm, his left arm being off near left shoulder joint, and advanced age, his age being now about 62 years and tat such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, information and belief all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife to my knowledge is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That he was a Confederate soldier. Belonging to Company "G" 41st Regiment of Ala. Infty. Vols. That as such soldier he served from 1862 - 1863 to the close of the war in the spring of 1865. That he was honorably released from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of gun shot wound in left arm, his left arm being off close to shoulder, and old age and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. I, I.K. Leming a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with Thomas M. Sudduth of Waldron, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound or disability: left arm is off near the shoulder and he is suffering with nervous [illegible] and General disability.
Physical condition and to what cause is his incapacity for manual labor attributable: General Disability and loss of left arm and that said disability is not the result of his own vicious habits still persisted in.

I, Thomas M. Sudduth do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex- Confederate soldier, under an Act of the General Assembly of 1901, having made my original application from the county of Scott; that I am still in indigent circumstances; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that my disability has increased and I am less able to perform manual labor than when my original application was made, by reason of increased age and increased disability. I am 68 years old and I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

I, J.O.A. Sullivan a duly registered and practicing physician in the town of Waldron, Scott County, Arkansas, do hereby certify that I am personally well acquainted with Thomas M. Sudduth of Waldron, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physician [sic] condition and find: State description and character of wound: left arm off at shoulder also bad lung and Bronchial trouble and General debility.
