APPLICATION FOR PENSION
To the Honorable County Court of Scott County, [Blank] sitting as a Pension Board:
The undersigned, as an ex-Confederate soldier, hereby applies for a pension under the provisions of an act of the General Assembly of the State of Arkansas,
entitled "An Act for the Relief of Certain Soldiers of the late War between the States," approved April 1, 1891, upon the following grounds:
That he is a bona fide citizen of the State of Arkansas and has resided within this State for more than twelve months next before the filing of this application;
That he enlisted as a soldier in the State of Arkansas, during the war between the States in Company "K" of the Col. King's Regiment of Infantry Vols.
That while in the discharge of his duty as a soldier of said company and regiment in the service of the State of Arkansas or of the Confederate States of America,
he received a wound, from the effects of which he lost the use partially of his left leg, a leaden bullet from the ranks of the enemy striking him just below left knee
breaking and crushing bone - which wound has always hurt and hindered him from performing manual labor. The wound was received at the Battle of Prairie Grove.
That by reason of said wound and disability he is now indigent and incapacitated for manual labor;
That he is not receiving any aid from this State or from the United States, for such injuries, under any other statute, and that therefore he makes application for
relief under the act aforesaid.

examined the application of Geo. W. Rogers, within named, for pension, under Act of the General Assembly of the State of Arkansas as approved April 1, 1891 and the proof in support
of same, and find that said applicant is a wounded Confederate soldier, is in indigent circumstances and partially incapacitated for manual labor, and that his claim is just, and that
he should be allowed a pension and we hereby allow the sum of forty Dollars per annum.

one to serve one year is hereby honorably discharged from the Army of the Confederate States, in consequence of a wound received in Prairie Grove Battle [one line illegible]
in the State of Tenn is 20 years of age 5 feet 6 inches high, Dark complexion, Black eyes, Black hair, and by occupation when enlisted, a Farmer.
Given under my hand at Little Rock this 5th day of May in the year of our Lord one thousand eight hundred and sixty three.

I, H.T. Manning, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with George W. Rodgers of Boles,
Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his wound and find:
State description and character of wound. Gunshot in upper part of left leg. Damaging the head of Tibular [illegible] that it is [illegible] and greatly [illegible.]
Physical condition and to what cause is his incapacity for manual labor attributable
General health good. Incapacity for labor attributable to above described wound.

APPLICATION FOR PENSION

I, George W. Rogers of Boles, Ark. do solemnly swear that I served as a soldier in the army of the Confederate States, being a member of Company "K" in Col. Kings Regiment of Infantry
Vols. from the State of Arkansas. That I enlisted in said service on Nov. 22, 1861; that I was honorably discharged from such service at the close of the war on or about the 5th day
of May 1863 on account of wound received in Prairie Grove Battle and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this
State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00
(exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt
of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the
ordinary avocations of life, and that such incapacity (or disability) is the result of wounds received in the service, being gun shot wound in left leg, the bullet striking him just below
left knee joint breaking and crushing bone, also from diseases of rheumatism and neuralgia, and from advanced age, my age now being 66 years, and consequent debilitated physical
condition, and that such disability is not the result of my own vicious habits still persisted in, so help me God.

I, Dr. W.L. Tolleson a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with George W. Rogers of Boles,
Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound or disability: Gun shot wound of his left knee and
broken thigh.
Physical condition and to what cause is his incapacity for manual labor attributable: to gun shot wound of left knee and left thigh and [faded and illegible] and that said disability
is not the result of his own vicious habits still persisted in: they are due to any vicious habits
Extent of disability: He is incapacitated to perform manual labor to amount of one half

On this day personally came before the undersigned Justice of the Peace within and for the County of Scott and State of Ark., the undersigned two citizens of Scott Co., Ark. whom I
certify to be creditable persons and worthy of confidence, who being duly sworn, state that they are each, personally well acquainted with applicant Geo. W. Rogers and have known him
for 13 years, respectively. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age and
poor health and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge, all property now owned by him
and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel.) That neither he nor his wife is in receipt of any income, annuity, pension or
wages for any services or the emoluments of an office, in excess of $150 per year. That we have no interest in this claim.

I, G.W. Rogers, do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex-Confederate
soldier, under an Act of the General Assembly of 1901, having made my original application from the County of Scott; that I am still in indigent circumstances; that I do not myself,
nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods
and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension
or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that
my disability has increased, and I am less able to perform manual labor than when my original application was made, by reason of not being able to do any manual labor of any kind and
I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

was at the time a pension was granted him, and that he is entitled to have the same increased to 100 Dollars.

I, J.O.A. Sullivan, a duly registered and practicing physician in Waldron, Scott County, Arkansas, do hereby certify that I am personally well acquainted with G.W. Rogers of Chant,
Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: Shot below knee of left leg. Bones having been cut off
and badly put together making him a bad cripple, his age is 77 years.
Physical condition and to what cause is his incapacity for manual labor attributable: to the above causes (which is bad) and that said disability is not the result of his own
vicious habits still persisted in: No
Extent of disability Total. He is not physically able to do work of any kind.
