APPLICATION FOR PENSION

To the Honorable County Court of Scott County, Sitting as a Pension Board:
The undersigned, as an ex-Confederate soldier, hereby applies for a pension under the provisions of an act of the General Assembly of the State of Arkansas, entitled "An Act for the Relief of Certain Soldiers of the late War between the States," approved April 1, 1891, upon the following grounds:
That he is a bona fide citizen of the State of Arkansas and has resided within this State for more than twelve months next before the filing of this application;
That he enlisted as a soldier in the State of Tennessee during the war between the States in Company F of the 12 Regiment of Tennessee Volunteers.
That while in the discharge of his duty as a soldier of said company and regiment in the service of the State of Arkansas, or of the Confederate State of America, he received a wound, from the effects of which he lost the partial use of right leg.
That by reason of said wound and disability he is now indigent and incapacitated for manual labor; That he is not receiving any aid from this State or from the United States, for such injuries, under any other statute, and that therefore he makes application for relief under the act aforesaid.

On this day personally came before the undersigned a Notary Public within and for the County of Sebastian and State of Arkansas, J.M. Stephens and Robt. R. Pace citizens of Huntington, Arkansas, whom I certify to be credible persons and worthy of confidence who being duly sworn, state that they are each, personally, well acquainted with the applicant D.C. Johnson and have known him Forty years.
That the material facts set forth in his application for pension are true. That he was a Confederate soldier. Belonged to Company "F" 12th Regiment of Tenn. Vols. That he was wounded in said service at Shilough, Tennessee in right leg. That this information we derive from the following sources:
From long acquaintance with said soldier and frequently hearing him speak of his Confederate service - and knowing that on account of a wound in right leg said soldier is unable to perform manual labor.
That he is partially incapacitated for manual labor, and that we have no interest in this claim.

I, H.J. Hodges, a duly registered and practicing physician in Scott County, Arkansas, do certify that I am acquainted with D.C. Johnson, an applicant for pension under the Statutes of Arkansas, and that he is incapacitated for manual labor by reason of a wound in the right leg, and by reason of said wound he is in indigent circumstances.

VETERAN'S APPLICATION FOR PENSION

STATE OF ARKANSAS}
County of Scott}
I, D.C. Johnson do solemnly swear that I served as a soldier in the army (or sailor in the navy) of the Confederate States, being a member of Co. F, 12th Regiment of Tenn. Inf. from the State of Tennessee; that I was honorably discharged (paroled or released) from such service on or about the 15th day of April 1865 and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $500.00 (not including the value of homestead, or household goods, owned by either or both); nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $250.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life (or am totally blind), and that such incapacity (or disability) is the result of wounds received in the service, being Gun shot wound in right leg and that my age together causes my disability to labor or of age, accident or disease, and that such disability is not the result of my own vicious habits still persisted in, so help me God.

On this day personally came before the undersigned, a Notary Public within and for the County of Scott and State of Arkansas, J.R. Phillips, a citizen of Sebastian County whom I certify to be a credible person and worthy of confidence, who, being duly sworn, states that he is personally well acquainted with applicant D.D. Johnson and has known him for 60 years.
That he was a Confederate soldier belonging to Company [Blank] 12th Regiment of Tennessee Infantry.
That as such soldier he served from 1861 to Close of war. That he was honorably discharged, paroled or released from such service and did not desert the same.
That he has been for the past twelve months a bona fide resident of Arkansas. That my knowledge of these facts comes from my being with him as a soldier in time of the war, and belonging to the same regiment he did and for the last several years have lived in the same community he now lives in, and he is not able to do manual labor.
That to the best of my knowledge all property owned by him is not worth exceeding $500.00, not including the value of his homestead,* or household goods. That he is not in receipt of any income, annuity, pension or wages for any services in excess of $250.00 per year. That we have no interest in this claim.

PROOF OF SERVICE AND OF INDIGENCY

On this day personally appeared before the undersigned, a Notary Public within and for the County and State aforesaid, T.F. Reed, a citizen of Sebastian County, State of Arkansas, whom I certify to be a credible person and worthy of confidence, who being duly sworn states, that he is personally well acquainted with the applicant, D.C. Johnson and have known him personally for about 57 years.
That he was a confederate soldier. I was not in the army with him, but we lived in the same community that he did in Tenn. and I remember that he was wounded in army and came home. I do not remember the year.
That by reason of his age he is incapacitated to do manual labor, and that such incapacity is not the result of his vicious habits still persisted in.
That to the best of my knowledge and belief all property owned by him and his wife, together, is not worth exceeding Five Hundred Dollars (exclusive of household goods, wearing apparel and the Homestead). That neither he nor his wife have disposed of any property to my knowledge for the purpose of drawing a pension.
That they are neither in receipt of any annuity or income, pension or wages for any service or the emoluments of an office in excess of two hundred dollars per year.
That I have no interest in this claim.

Johnson for pension, under Act of General Assembly of the State of Arkansas, as approved March 11, 1901, and subsequent amendatory Acts, and the proof in support of same, and find that said applicant is a wounded Confederate soldier, is in indigent circumstances, and wholly or partially incapacitated for manual labor, and that his claim is just, and that he should be allowed $100.00 pension.

Duncan Clerk, sitting as a Pension Board for Scott County, do certify that we have examined the application of the within named D.C. Johnson for pension, under act of the General Assembly of the State of Arkansas, as approved April 1, 1891, and the proof in support of same, and find that said applicant is a wounded Confederate Soldier, is in indigent circumstances and partially incapacitated for manual labor, and that his claim is just, and that he should be allowed a pension and we further certify that we believe said soldier is entitled to the sum of Twenty five Dollars per annum [illegible] This July 18, 1893.
