APPLICATION FOR CONFEDERATE PENSION

STATE OF ARKANSAS}
County of Scott}
To the Honorable County Court of Scott County:
The undersigned applicant for pension under an act of the General Assembly of the State of Arkansas, approved April 1, 1891, and amendments thereto, being duly sworn according to law, states that he is the identical M.J. Hendricks who enlisted and was enrolled on the first day of September 1862 in Company K of the 27th Regiment of Mississippi Vol. commanded by Col. Cambell in the services of the Confederate States of America, or the State of Arkansas. That while a member of the said Company and Regiment in said service, and in the line of his duty at Chicamauga in the State of Tennessee on or about the 19th or 20th day of September 1863 he received a gun shot wound in the hand and lost two fingers and from the explosion of a shell he was burned in the eye. That by reason of said wound he lost two fingers on right hand and has lost his eye cite [sic] almost entirely and is now in indigent circumstances, without means of support, and is wholly incapacitated for manual labor. That he is a bona fide citizen of the State of Arkansas, and has resided in said State for more than twelve months immediately preceeding this application. That he is not receiving aid from the State of Arkansas, from the United States nor from any other State, for injuries received in the war between the States under any Statute providing for the relief of the maimed and blind soldiers of the States.

We, E.D. Yandell, Judge of the County Court of Scott County, L.P. Fuller, Sheriff and T.M. Duncan, Clerk, sitting as a Pension Board for Scott County, do certify that we have examined the application of the within named M.J. Hendricks for pension, under act of the General Assembly of the State of Arkansas, as approved April 1, 1891, and the proof in support of same, and find that said applicant is a wounded Confederate soldier, is in indigent circumstances and partially and almost wholly incapacitated for manual labor, and that his claim is just, and that he should be allowed a pension and we hereby allow the sum of Fifty Dollars per annum.

On this day personally came before the undersigned, a Notary Public within and for the County of Sebastian and State of Arkansas, Jake Pickle and W.C. Blaylock, citizens of Hartford, Sebastian Co., Ark., whom I certify to be credible persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with the applicant M.J. Hendricks and have known him 35 years.
That the material facts set forth in his application for pension are true. That he was a Confederate soldier. Belonged to Company K 27th Regiment of Mississippi. That he was wounded in said service at Chicamauga Battle in Tennessee [Last four words lined through]
That this information we derive from the following sources: from long acquaintance with said soldier and often hearing him relate his army service and as comrades in said service and belonging to same army in the Confederacy.
That he is partially and almost wholly incapacitated for manual labor, and that we have no interest in this claim.

EVIDENCE OF PHYSICIAN

I, J.A.J. Roth, a duly registered and practicing Physician in Scott County, Arkansas, do certify that I am personally acquainted with M.J. Hendricks, an applicant for pension under the Statutes of Arkansas, and that he is incapacitated for manual labor by reason of a wound in the Hand and is almost totally blind, and by reason of said wound he is in indigent circumstances.

EVIDENCE OF PHYSICIAN

I, J.F. Bentley, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with M.J. Hendricks of Gipson, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his wound and find: State description and character of wound: I find a wound on left side head, said to be received from explosion of a shell.
Physical condition and to what cause is his incapacity for manual labor attributable: Applicant is now suffering from loss of eye-sight said to have been caused from the above wound.
Financial condition: None. Suporting has no property[?]
Means of support: None
Extent of disability: Totally disabled.
