APPLICATION FOR PENSION
To The Honorable County Court of Scott County:
Sitting as a Pension Board:
The undersigned as an ex-Confederate soldier, hereby applies for a pension under the provisions of an act of the General Assembly of the State of Arkansas, entitled "An Act for the
Relief of Certain Soldiers of the late War between the States," approved April 1, 1891, upon the following grounds:
That he is a bona fide citizen of the State of Arkansas, and has resided within this State for more than twelve months next before the filing of this application; That he enlisted as a
soldier in the State of Arkansas, during the war between the States in Company "F" of the 1st Reg't. Ark. Infty. Vols.
That while in the discharge of his duty as a soldier of said company and regiment in the service of the State of Arkansas, or of the Confederate States of America, he received a wound
from the effects of which he lost the use of his right foot, the leaden ball passing through right ankle causing stiffness of ankle and foot which greatly disables him from performing
any kind of labor. That his wound was received at the battle of Chickamauga. That by reason of said wound and disability he is now indigent and incapacitated for manual labor; That he
is not receiving any iad from this State or from the United States, for such injuries, under any other statute, and that therefore he makes application for relief under the act
aforesaid.

AFFIDAVIT
State of Arkansas}

In matter of Pension claim of Henry R. Goodwin, Private Co. "F" 1st Reg't. Ark. Infty. Vols. [word] 25th day of Sept. 1891 before the undersigned Clerk of the Circuit Court personally
appeared G.M. Grandstaff and W.P. Forrester who each being first duly sworn in regard to the pension claim of the above named ex-Confederate soldier say: That they have each been
acquainted with said soldier for the past six years; that he is the identical person he represents himself to be. That said soldier is wounded in right foot, that his right foot and
ankle are stiff. That we have each seen the wound often and have been in company with said soldier a great deal since we first became acquainted with him and know that he is not able
to perform manual labor by reason of said injury. Soldier has stated to us frequently that said injury of his right foot was received while in the discharge of his duty as a soldier in the
aforesaid company and regiment. Claimant is now indigent and incapacitated for manual labor by reason of said injury. Affiants further state that they have no interest whatever in
this claim.
Affiant G.M. Grandstaff says that his age is 34 years.
Affiant W.P. Forrester says that his age is 37 years.

We, James M. Harvey, Judge of the County Court, and L.P. Fuller, Sheriff, and Thomas M. Duncan, County Clerk of Scott County, constituting the Pension Board of said County, by Act of
the General Assembly of the State of Arkansas, approved April 1, 1891, entitled "An Act for the Relief of certain Soldiers of the Late War Between the States," do hereby certify to
the Auditor of the State of Arkansas that upon examination of the application of Henry R. Goodwin filed under said Act, we find that said claim of said applicant is just and the
same is hereby allowed, the application and proof taken in said case being herewith transmitted.
Given under our hands and the Seal of the County Court of Scott County, on this 25th day of Sept. 1891.

APPLICATION FOR PENSION

I, Henry R. Goodwin do solemnly swear that I served as a soldier in the army of the Confederate States, being a member of Co "F" commanded by Capt. W.H. Martin of Col. Jos. F.
Fagans 1st Regiment of Infty. Vols. from the State of Arkansas; that I was honorably paroled from such service on or about the [blank] day of June 1865 and did not desert the same;
that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am a widower, and that I do not myself own property, real or personal, or both,
or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor have I conveyed title to any property to enable me to draw
a pension, and that I am not in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated
to perform manual labor in any of the ordinary avocations of life, and that such incapacity (or disability) is the result of a wound received in the service, being minie ball wound
in right ankle joint which has troubled and pained me during the whole time since the war to the present time, my right ankle joint being stiff, and which wound so received in the
service has greatly disabled me and from advanced age, my age now being 61 years, and subsequent debilitated physical condition and that such disability is not the result of my
own vicious habits still persisted in, so help me God.

General Assembly of the State of Arkansas, as approved March 11, 1901, and the proof in support of same, and find that said applicant is one half disabled a wounded Confederate soldier,
is in indigent circumstances, and wholly or partially incapacitated for manual labor, and that his claim is just, and that he should be allowed $50.00 pension.

EVIDENCE OF PHYSICIAN

I, J.O.A. Sullivan a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with Henry R. Goodwin of Parks, Scott Co.,
Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound or disability: I find that the soldier was shot through
the right ankle with a minie ball of large size crushing the joint entirely.
Physical condition and to what cause is his incapacity for manual labor attributable: Making the joint stiff and also the joints of the foot and toes I find from this wound that the
leg to the knee joint has perished away and the circulation is very bad in said limb. These conditions exist from said wound and not from any vicious habits.
Extent of disability: 1/2

On this day personally came before the undersigned, a Notary Public within and for the County of Faulkner and State of Arkansas, A.D. Haynie, a citizen of Cato, Ark., whom I certify to be a
creditable person and worthy of confidence, who being duly sworn, states that he is personally well acquainted with applicant Henry R. Goodwin, having known him for 35 years.
That he was a Confederate soldier. Belonging to Company "F" in 1st Regiment of Ark. Infty. Vols. That as such soldier he served from 1861 to the close of the war in spring of 1865.
That he was honorably paroled from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is
incapacitated for manual labor by reason of advanced age and consequent debilitated physical condition, and from wound in right ankle joint received in service and that such incapacity
(or disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, all property now owned by him and his wife, together, is not worth
exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the
emoluments of an office, in excess of $150 per year. That I have no interest in this claim. I testify to above facts from following sources: I knew soldier during the war and knew that
he was serving in the Confederate Army, although I belonged to a different company and regiment.

30 years.
That he was a Confederate soldier. Belonging to Company "F" 1st Regiment of Ark. Infty. Vols. That as such soldier he served from spring of 1861 to the close of the war in spring
of 1865. That he was honorably paroled from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That
he is incapacitated for manual labor by reason of wound in right ankle received in service and old age and consequent debilitated physical condition and that such incapacity (or
disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, all property now owned by him and his wife, together, is not worth
exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the

APPLICATION FOR INCREASE OF PENSION

I, H.R. Goodwin do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex-Confederate
soldier, under an Act of the General Assembly of 1901, having made my original application from the county of Scott; that I am still in indigent circumstances; that I do not myself,
nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and
wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension
or wages for any services, the emoluments of any office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that
my disability has increased, and I am less able to perform manual labor than when my original application was made, by reason of increased age and increased disability. I am 69 years
of age and I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

We, the undersigned, sitting as a Pension Board for Scott County, do hereby certify that we have examined the application of the within named H.R. Goodwin for increase of pension,
under Act of the General Assembly of the State of Arkansas, approved March 9, 1907, and the proof in support of same, and find that said applicant is less able to perform manual
labor than he was at the time a pension was granted him, and that he is entitled to have the same increased to 100 Dollars.

I, Lucien K. Robertson, a duly registered and practicing physician in Parks, Scott County, Arkansas, do hereby certify that I am personally well acquainted with H.R. Goodwin of
Parks, Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: Gunshot wound in right ankle joint. Stiffening completely
that joint with loss of use of foot (partially) also leg.
Physical condition and to what cause is his incapacity for manual labor attributable From wound of said ankle he's unable to walk sufficient to do work on farm and that said
disability is not the result of his own vicious habits still persisted in.
Extent of disability: Total
