APPLICATION FOR PENSION

STATE OF ARKANSAS}
County of Scott}
To the Honorable County Court of Scott County
The undersigned applicant for a pension under an act of the General Assembly of the State of Arkansas, approved April 1st, 1891, and amendments thereto, being duly sworn, states that his name is M.C. Frizzell, his age is 53 years, that he has resided in this State for 43 years, that he is not receiving aid from the State of Arkansas or any other State or government for injuries received in the war between the States.
That he enlisted at Waldron, Scott Co., Ark. on or about the Sept. 1861 in Company H, commanded by Capt. Torbit in the 19 Regiment of Ark. Infantry commanded by Col. Dawson in Confederate State Service. Afterwards re-enlisted in Company [blank] in the [blank] Regiment of [blank] commanded by [blank] in same service. That while a member of said Company in said service and while in the line of duty at Kennisaw Mountain, in the State of Georgia, or or about the 22 day of June 1863 he received a Gun shot wound. (Describe the kind, character and resulting consequences of wound or wounds.) The wound was received in right arm above elbow joint which caused the arm to be disarticulated at the shoulder joint and four inches of the bone removed.
My means of support are about as follows:
I have a farm and am depending upon it for a living but am unable to do a days work.

Duncan, Clerk, sitting as a Pension Board for Scott County, do certify that we have examined the application of the within named M.C. Frizzell for pension, under act of the General Assembly of the State of Arkansas, as approved April 1, 1891, and amendments thereto, and the proof in support of same, and find that said applicant is a wounded Confederate soldier, is in indigent circumstances and partially incapacitated for manual labor, and that his claim is just, and that he should be allowed $30 pension.

EVIDENCE OF PHYSICIAN

STATE OF ARKANSAS}
County of Sebastian}
I, J.W. Sorrels a duly registered and practicing physician in Sebastian County, Arkansas, do hereby certify that I am personally well acquainted with M.C. Frizzell of Scott County, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his wound and find: State description and character of wound: The humarus has been disarticulated at sholder joint and about four inches of the bone removed.
Physican condition and to what cause is his incapacity for manual labor attributable: His physical condition is good - except his arm, the condition (weakness) renders him unable to perform manual labor.
Financial condition: He has a farm and team sufficient to runn it, but states that he is in debt.
Means of support: From farm above.
Extent of disability: Equal to loss of an arm.

VETERAN'S APPLICATION FOR PENSION

I, M.C. Frizzell do solemnly swear that I served as a soldier in the army (or sailor in the navy) of the Confederate States, being a member of Company H, Regiment of 19th Arkansas from the State of Arkansas; that I was honorably discharged (paroled or released) from such service on or about the 22 day of June 1864 and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $500.00 (not including the value of homestead, or household goods, owned by either or both); nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $250.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life (or am totally blind), and that such incapacity (or disability) is the result of wounds received in the service, being a Gun Shot wound in wright sholder which I Received in the "Civil War" on the 22nd Day of June 1864 which Disabled me of Doing manual labor or of age, accident or disease, and that such disability is not the result of my own vicious habits still persisted in, so help me God.

I, Dr. J.H. Stone, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with M.C. Frizzell of Abbott, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: He has a gun shot wound in his wright sholder joint which he received in the "Civil War" on the 22nd day of June 1864, which disables him of doing manual labor.
Physical condition and to what cause is his incapacity for manual labor attributable: too rheumatism and kidney trouble and that said disability is not the result of his own vicious habits still persisted in: NO
Extent of disability: Two thirds disabled

Chitwood and M.A. Looper, citizens of Scott and Sebastian County whom I certify to be credible persons and worthy of confidence, who, being duly sworn, state that they are each, personally, well acquainted with applicant M.C. Frizzell and have known him Fifty or more years, respectively.
That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of no vicious habits of his own and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge all property now owned by him and his wife, together, is not worth exceeding $500.00, not including the value of his or her homestead,* or household goods. That neither he nor his wife is in receipt of any income, annuity, pension or wages for any service, or the emoluments of an office, in excess of $250.00 per year. That we have no interest in this claim.

Under authority of a ruling of the State Pension Board and Advisory Board, appointed by the Governor, at a meeting of said Boards on the 7th day of January, 1932.

8. Do you own any property, or interest in any property? Yes

CERTIFICATE OF COUNTY CLERK
(Of Pension Due Estate of Deceased Pensioner)

I, Gene Davidson, Clerk of the County Court within and for the County and State aforesaid, do hereby certify that M.C. Frizzell was a Confederate pensioner, residing in said State, that the application of the said M.C. Frizzell was originally filed in said County; that said M.C. Frizzell died on or about the -- day of September, 1934; that the last allowance or issuance of a pension warrant to said deceased pensioner was made in August, 1934, and that O.S. Bird is the duly acting Administrator of his said estate, and that he is entitled to receive, under the provisions of Section 8193 of Crawford & Moses' Digest, and to be disbursed by him under authority of Section 8194 of said Digest, the pro rata amount accruing from August, 1934, to date of death of said pensioner, subject to the apportionment of the pension fund made by the State Board of Pensioners for the present year.
