APPLICATION FOR PENSION

STATE OF ARKANSAS}
County of Scott}
I, J.W. Chrisman do solemnly swear that I served as a soldier in the army (or sailor in the navy) of the Confederate States, being a member of Hill's Regiment Co. A Regiment of Mounted Infantry from the State of Arkansas or a member of the crew of the ship called [blank]; that I was honorably discharged (paroled or released) from such service on or about April 1865 and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any service, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life (or am totally blind), and that such incapacity (or disability) is the result of wounds received in the service, being wounded at Pilot Nob, Mo., and account of old age and disability or of age, accident, or disease, and that such disability is not the result of my own vicious habits still persisted in, so help me God.

EVIDENCE OF PHYSICIAN

STATE OF ARKANSAS}

I, J.O.A. Sullivan, a duly registered and practicing physician in the town of Waldron, Scott County, Arkansas, do hereby certify that I am personally well acquainted with J.W. Chrisman of Waldron, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: in right thigh bullet resting against the femer by [illegible] to some extent from said wound. Wound on left side of head sabre cut causes a good deal of pain in head, has caused deafness almost complete.
Physical condition and to what cause is his incapacity for manual labor attributable: to the above causes and his age, his leg giving way when he [illegible] or works much. and that said disability is not the result of his own vicious habits still persisted in: Habits good
Extent of disability: One half 1/2

Castleberry, citizen of Scott County, Ark. whom I certify to be [a] creditable person and worthy of confidence, who, being duly sworn, sthat that they are each, personally, well acquainted with applicant J.W. Chrisman and have known him 18 years. I have heard him talk often about having been in the Confederate army and having been wounded in the service. I have heard him have such talk every since I have known him, but do not of my own knowledge that he was such soldier. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age and general disability and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That we have no interest in this claim.

That he was a Confederate soldier as I verily believe although I have no personal knowledge of the fact. I believe him to have been a soldier from frequent conversation with him and from hearing him narate [illegible] which he could not have known had he not been in the service. That he is now and has been for the past twelve months a bona fide resident of the State of Arkansas. That he is incapacitated for manual labor by reason of old age and general disability and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That I have no interest in this claim.

Chrisman and have known him 43 years, respectively.
That he was a Confederate soldier. Belonging to Company "A" Hill's Regiment of Mounted Infantry. That as such soldier he served from about 1863 to about April 1865. That he was honorably discharged (paroled or released) from such service and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That we have no interest in this claim.

APPLICATION FOR INCREASE OF PENSION

I, J.W. Chrisman, do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex-Confederate soldier, under an Act of the General Assembly of 1901, having made my original application from the County of Scott; that I am still in indigent circumstances; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that my disability has increased, and I am less able to perform manual labor than when my original application was made, by reason of increased age and increased general disability. I am about 72 years of age according to the best information I can get, but do not know my exact age and I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

Chrisman for increase of pension, under Act of the General Assembly of the State of Arkansas, approved March 9, 1907, and the proof in support of same, and find that said applicant is less able to perform manual labor than he was at the time a pension was granted him, and that he is entitled to have the same increased to 100.00 Dollars.

I, J.O.A. Sullivan a duly registered and practicing physician in Waldron, Scott County, Arkansas, do hereby certify that I am personally well acquainted with J.W. Chrisman of Waldron, Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: Severe wound of the Head on the left side causing the ear to run causing great and constant pain & also wound in right thigh causing the muscle to perish. Wound in right knee.
Physical condition and to what cause is his incapacity for manual labor attributable: To the above named wounds and that said disability is not the result of his own vicious habits still persisted in Has no bad habits.
Extent of disability: Total

WIDOW'S APPLICATION

I, Louisa Chrisman do solemnly swear that I was born the 20th day of November 1871 and that I am the widow of J.W. Chrisman, who served as a soldier in the army (or sailor in the navy) of the Confederate States, his proof of service being now on file in the Auditor's office, at Little Rock, he having been allowed a pension under Act of the General Assembly, approved March 11, 1901, and subsequent amendatory Acts, while a resident of Scott County, Arkansas; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not own property, real or personal, or both, or money or choses in action, in excess of the value of $500.00, not including the value of homestead, or household goods; nor have I conveyed title to any property to enable me to draw a pension, and that I am not in receipt of any income, annuity, pension or wages for any service, the emoluments of an office, in excess of $250.00 per year; that my husband died on the 3rd day of April 1915, and that I have not since remarried, so help me God.
