We, the undersigned, sitting as a Pension Board for Scott County, do certify that we have examined the application of the within named George W. Burns for pension, under Act of General Assembly of the State of Arkansas, as approved March 11, 1901, and the proof in support of same, and find that said applicant is an injured and disabled Confederate soldier, is in indigent circumstances, and partially incapacitated for manual labor, and that his claim is just, and that he should be allowed $25.00 pension.

APPLICATION FOR PENSION

I, George W. Burns do solemnly swear that I served as a soldier in the army of the Confederate States, being a member of Col. Avery's 4th (Co. "A") Regiment of Cavalry Vols. from the State of Georgia; that I was honorably paroled or released from such service on or about the [blank] day of May, 1865 at the close of the war and did not desert the same; that I am now and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, inexcess of $150.00 per year; that I am incapacitated to perform manual labor in any of the oridinary avocations of life and that such incapacity (or disability) is the result of advanced age, my age being now about 55 years, and rupture of right side and consequent debilitated physical condition or of age, and that such disability is not the result of my own vicious habits still persisted in, so help me God.

I, J.O.A. Sullivan, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with G.W. Burns of near Waldron, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound or disability: The soldier is badly ruptured in right side which he states was done while in the armie; he also had his right arm broken about one year ago.
Physical condition and to what cause is his incapacity for manual labor attributable: [Blank] and that said disability is not the result of his own vicious habits still persisted in [Blank]

Satterfield, a citizen of Scott Co., Ark., whom I certify to be a creditable person and worthy of confidence, who being duly sworn, states that he is personally, well acquainted with applicant G.W. Burns, and have known him for 40 years.
That he was a Confederate soldier. Belonging to 4th Ga. Cavalry. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age, and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel.) That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That I have no interest in this claim. That I was personally acquainted with soldier when the war commenced and I understood at the time and it was the general understanding among his neighbors that he joined the Confederate Army and I have always understood and heard the same since the war to the present.

Burns, having known him for about 35 years. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age, he being now about 54 years old and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of my knowledge, all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel.) That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That I have no interest in this claim. That I became acquainted with soldier after the close of the war - and it was my information and understanding then and has always been since that he belonged to the Confederate Army - although I did not see him in the service - my son-in-law W.L. Dole servedin the same Company with him (Dole)

Tate, Jeremiah Burns, citizens of Walker, Georgia, whom I certify to be creditable persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with applicant George W. Burns and have known him for 40 years, respectively.
That he was a Confederate soldier. Belonging to Company "A" in Col. Avery's 4th Regiment of Ga. Cavy. Vols. That as such soldier he served from about April 1864 to the close of the war about month of May, 1865. That he was honorably paroled or released from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of advanced age and resulting debilitated physical condition and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. Burns and have known him for 10 years and 8 years, respectively.
That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of advanced age, and consequent debilitated physical condition and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge, all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. I, J.O.A. Sullivan, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with G.W. Burns of Waldron, Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: right arm has been broken and is some atrified. Broken between the elbow and shoulder. His age is 61, suffers with rheumatism.
Physical condition and to what cause is his incapacity for manual labor attributable: To the above named causes and rupture and that said disability is not the result of his own vicious habits still persisted in: No

Burns do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex-Confederate soldier, under an Act of the General Assembly of 1901, having made my original application from the county of Scott; that I am still in indigent circumstances; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that my disability has increased, and I am less able to perform manual labor than when my original application was made, by reason of increased age and increased disability. I am 61 years of age and I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

I, I.K. Leming, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with Geo. W. Burns of Scott Co., Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound - rupture of right side and inflamation of kidneys and bladder.
Physical condition and to what cause is his incapacity for manual labor attributable: rupture and kidney trouble and Rheumatism. He is sixty seven (67) years old and feeble. His wife is sixty nine (69) years old and almost helpless - and with no visible means of support and that said disability is not the result of his own vicious habits still persisted in [Signed] I.K. I, Elizabeth Burns do solemnly swear that I was born the 10 day of November 1859 and that I am the widow of George W. Burns, who served as a soldier in the army (or sailor in the navy) of the Confederate States, his proof of service being now on file in the Auditor's office, at Little Rock, he having been allowed a pension under Act of the General Assembly, approved March 11, 1901, while a resident of Scott County, Arkansas, that I am now, and for the past twelve months have been a bona fide resident of this State; that I do not own property, real or personal, or both, or money or choses in action, in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor have I conveyed title to any property to enable me to draw a pension, and that I am not in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that my husband died on the [blank] day of Oct. 1916 and that I have not since remarried, so help me God.
